If you would like to request deletion of your information, please read the below, then complete and submit your deletion request form.

Please note that the right to deletion under the law is not absolute, and there are exceptions to this right under CA law.

Prior to deleting any personal information, we need to verify your identity.

In order to protect your information and privacy, we need you to confirm your identity.  You may not request deletion of someone else’s personal information unless they have given you the authority to do so.
